How do I form the passive voice in German?

Conjugated form of werden + Partizip II of the main verb at the end of the sentence.

Example

Active: Der Mann liest das Buch. (The man reads the book.)

Passive: Das Buch wird gelesen. (The book is being read.)

Passive (with agent): Das Buch wird vom Mann gelesen. (The book is being read by the man.)
